This role will directly manage a team of band 6 nurses, paramedics and an HCA.The role supports the delivery of planned care within a virtual ward model and responsive visits to support patient care across Rennie Grove Peace Hospice (RGP). The virtual ward is based at Peace Building in Watford with a plan to further expand the virtual ward across the RGP NW Herts community locality.Main duties of the jobThis Means You Will Be Responsible ToActively participate in the development of the organisations planned and unplanned community model of care. Offer advice, support and education to the primary health care team as part of an interdisciplinary team. The management of a specialist palliative virtual ward caseload across the RGP Watford, 3 Rivers and Hertsmere locality and support other RGP virtual wards within Hertfordshire as required to maintain our service. To work from an interdisciplinary approach to care. To engage in the full range of activities consistent with the role of a service lead. Demonstrate specialist skills and expertise in palliative care, the post holder will deliver high standards of personalised care. To demonstrate the ability to prioritise and reprioritise is an essential in this role. Improve patient outcomes and experience by providing physical and emotional support to their patients, coordinating their care services, and informing and advising them on clinical and practical issues associated with their illness. Participate activities such as audits and case reviews to improve the Hospice @Home service As required, work across Rennie Grove Peace Hospice Care geographical area to support safe and effective service delivery. Proven experience of effective team working IT literate including a good knowledge of Excel, Word, a clinical IT system. Excellent written and verbal communication skills Good organisational skills Teaching and presentation skills Advanced assessment skills and specialist symptom control knowledge to plan, implement and evaluate nursing care Drive and enthusiasm with a positive approach Ability to work autonomously and as an effective member of the team Caring and empathetic approach Commitment to the organisation values Sensitivity and understanding to the needs of your team and customers, many of whom may have experienced Rennie Grove and Peace Hospice Care services or be bereaved.
