This team is dedicated to revolutionising healthcare delivery for frail patients by providing high-quality, patient-centred care in their own homes, reducing hospital admissions and length of stay. Role DetailsThis position is within the Medicines Management Team. Key responsibilities include working as an Advanced Clinical Pharmacist, managing medicines for high-risk patients, conducting clinical medication reviews in patients' homes, and collaborating with a multidisciplinary team. The role involves managing your caseload, providing pharmaceutical expertise to various healthcare professionals, and evaluating the service for continuous improvement. The base is at Parsons Green, but the role primarily involves working in Barnet, with lone working and travel required. A car is necessary. The role offers flexibility and potential for remote working where appropriate.
