We are currently recruiting for a Care Coordinator to maintain and develop our domiciliary service at our Hounslow branch. Core Duties & Responsibilities as Care Coordinator:Roster and template care calls using an electronic system. Direct management of the care team, monitoring staff development through supervisions, appraisals, and training. Create, manage, and monitor customer care plans, ensuring staff rotas meet service users’ needs and aspirations. Ensure care assessments, care, and risk planning meet best practices and customer expectations. Collaborate with the Recruitment Team to ensure sufficient staffing levels. Provide care services to service users in emergencies, arrange emergency cover, and cover calls as needed. Answer telephones promptly, speaking politely and respectfully. Work alongside the Branch Manager to address quality standards, complaints, and comments promptly. Participate in the paid on-call rota. Implement measures to comply with CQC regulations and strive for the highest care standards.
