Main duties of the jobWe are seeking a Community Matron within the South Beds Localities to work within the Complex Care/Primary Care Home Teams providing long term condition and minor illness assessments for patients that meet the service two-hour response criteria. You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills. You will be an experienced Registered Nurse with top to toe assessment and independent prescribing skills with a background in minor illness or chronic disease management.
