Job summary This is a fixed-term 12-month appointment for a Clinical Pharmacist to join our established PCN pharmacy team across the Network Area. The role is based at various practices within the network, responding to practice needs. The post holder will act within their professional boundaries, providing clinical support within the PCN and collaborating with multidisciplinary teams in general practice. They will receive appropriate clinical supervision and work to integrate community and hospital pharmacy services, aiming to improve patient outcomes, enhance healthcare access, and manage workload effectively. Key responsibilities include managing elements of the quality and outcomes framework, participating in quality improvement initiatives, conducting clinical medication reviews for patients with complex polypharmacy across various settings, representing the PCN in strategic partnerships on medicines optimisation and quality improvement, and supporting general practice staff with prescription and medication queries. There is also potential to develop into a non-medical prescriber role if desired. Ensure practice integration with community and hospital pharmacy to optimise skill mix, improve patient outcomes, enhance healthcare access, and manage workload. Manage aspects of the quality and outcomes framework and participate in quality improvement activities. Perform patient-facing clinical medication reviews, especially for patients with complex polypharmacy, across various settings including surgeries, homes, and residential care, aligning with primary care workstreams. Represent the PCN to strategic partners on medicines optimisation and quality improvement initiatives. Support general practice staff with prescription and medication queries, offering clinical medicines advice. Develop towards a non-medical prescriber role if desired. Person Specification Qualifications (Essential): Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. Experience (Essential): Experience and awareness of common acute and long-term conditions encountered in general medical practice.
