Job Description

We are looking to recruit an enthusiastic Clinical Assessor within the Cornwall Onward Care Team, based at Derriford Hospital. This role requires previous experience in patient flow.Opportunity Overview An exciting opportunity to support patient flow from an acute hospital, working alongside an experienced team. The aim is to achieve a collaborative approach to hospital discharge, ensuring patients stay in hospital only for the time required for their acute treatment and experience timely, seamless transfer of care to another setting.Key ResponsibilitiesAssessment Skills:Conduct holistic assessments to identify patient needs and support setting realistic discharge goals within the multidisciplinary team (MDT).Timely Evaluation:Ensure patient needs are assessed promptly and effectively.Communication & Liaison:Act as a link between agencies and departments to facilitate efficient communication and expedite onward care.Relationship Development:Build working relationships with ward teams to support patient flow across the trust.Collaborative Planning:Work with clinical teams, health and social care agencies, and the Integrated Transfer of Care Hub to plan and manage patient flow through the trusts.Follow-up & Care Planning:Contribute to follow-up assessments, care planning, resource identification, and service delivery for timely discharge.Flexibility & Proactivity:Work flexibly across organizational boundaries (RCHT, CFT, Adult Social Care, Voluntary Sector) responding to service needs.Communication Skills:Use highly developed communication skills in complex and potentially challenging situations.Staff Support & Guidance:Provide guidance, teaching, and support to other staff as appropriate.Advocacy & Conflict Resolution:Act as an advocate for patients and families, and actively resolve minor or potential conflicts, liaising with senior staff as needed.About Us We are an NHS community and mental health provider trust based in Cornwall and the Isles of Scilly, delivering community and hospital-based care to improve physical and mental health.
