About the RoleAre you an experienced Band 3 or Band 4 Therapeutic Assistant Practitioner seeking a new challenge? West Lothian continues to invest in community assessment and rehabilitation through our Home First programme. This initiative includes significant investment in community Allied Health Professions (AHP) teams and new funding to improve patient pathways via a community single point of contact. We are looking for experienced practitioners to join our growing team, delivering person-centered assessment and therapy across various community pathways, including pre-admission, discharge assessments, virtual community hospital wards, and step-down intermediate care facilities. Your role will involve collaborating with inpatient colleagues to facilitate early supported discharge and provide community rehabilitation.
