Job summary We have an exciting opportunity for an experienced senior clinician to provide operational leadership across NSFT for the new CMHT/community improvement programme, one of five priority programmes forming the new clinical transformation infrastructure led by Stewart Gee, Chief Transformation Officer. The main purpose of this programme is to deliver astandard service and workforce modelfor the adult CMHTs across NSFT, meeting local population needs, improving service interfaces, updating workforce models, and enhancing staff training and development, aligned with the NHS Long-Term Plan, Community Mental Health Services Framework (CMHSF), and other national initiatives. The service model will include physical healthcare and improvements for people with serious mental illness (SMI). The programme also addresses wider community services and interface issues with CMHTs, with some systems work led by the ICBs and supported by this role. The programme runs alongside Crisis improvement work and the inpatient delivery programme. A key aspect of the role is to lead on developing the service and workforce models for adult CMHTs, working closely with staff, service users, carers, and stakeholders including the ICBs, as outlined in the improvement plan. Main duties of the job You will be responsible for meeting actions assigned to this role as per the CMHT/community improvement plan, including aspects from the Crisis and Inpatient plans relevant to the community programme. Specific responsibilities include: Collaborating with BI/performance and the programme team to understand data, activity, and population health, establishing foundations for new operating and workforce models for adult CMHTs. Leading the development of the service model for adult CMHTs (to be completed by end Sept 25). Leading the physical healthcare model development for CMHTs (to be completed by end Sept 25). In addition, the role will: Have direct responsibility for the delivery of the CMHT/community programme across NSFT, ensuring alignment with strategic clinical networks. Provide updates and reports to relevant meetings as per governance and improvement plans. Ensure programme plans are current and prioritized accordingly.
