Main duties of the jobThe Role As an Advanced Clinical Pharmacist, you will be acornerstone of healthcare delivery and will play a critical role in our newmultidisciplinary team, covering two of the six new frailty virtual wards thatwe have planned for our locality. Here is what you will do Leadership within the Virtual ward regardingmedicines and medicines governance. Structured medication reviews includingcomplex polypharmacy and deprescribing. Prescribing and accurate record keepingregarding medications. Answering medication queries raised by themultidisciplinary team and providing clinical support to the Community FrailtyVirtual Ward Team. Work within integrated care across severalmulti-disciplinary teams including, Mental Health, District Nursing Discharge teams,Acute GPs, and Social Workers.
