You will contribute to service improvement projects, helping to shape and enhance care delivery for optimal outcomes. Main duties of the job As an Assistant Practitioner, you will manage your workload independently under supervision, supporting individuals with long-term conditions. Your responsibilities include providing evidence-based care, assisting in assessments, and supporting rehabilitation and self-management programs. Key Responsibilities: Delivering CareSupport initial consultations, implement, and modify care plans. Assist in rehabilitation sessions and weight management interventions. Promote person-centred care to enhance health and independence.Monitoring ProgressRecognize and respond to changes in physical or mental health. Provide feedback and contribute to assessments. Ensure safe, high-quality care and escalate concerns as needed.Mentoring Junior StaffSupport and supervise healthcare assistants and students. Provide guidance and foster a positive learning environment.
