The team sees patients in the Emergency Department to assess and identify patient who can safely be discharged or transferred rather than requiring an admission. They also provide therapy input input in our assessment units and care of the elderly wards aiming to progress patients and reduce length of stay.The team works in true partnership working with Social Care, the Older Peoples Mental Health team and community services identifying the most appropriate pathways for ongoing patient care.A flexible and pro-active approach is essential for this post as are excellent communication and team working skills.
