The team sees patients in the Emergency Department to assess and identify patient who can safely be discharged or transferred rather than requiring an admission. They also provide therapy input input in our assessment units and care of the elderly wards aiming to progress patients and reduce length of stay.The team works in true partnership working with Social Care, the Older Peoples Mental Health team and community services identifying the most appropriate pathways for ongoing patient care.A flexible and pro-active approach is essential for this post as are excellent communication and team working skills. You should have previous experience as a therapy assistant and be able to motivate patients throughout their journey.If you are caring, hardworking, self-motivated and wish to be part of our therapy team, we would like to hear from you.AdvertTo provide therapy assessment and treatment of patients under the guidance of a qualified Physiotherapist/Occupational Therapist To be responsible for the delivery of care within agreed guidelines and protocols To provide support for the overall Physiotherapy/Occupational Therapy service needs To be the first point of contact for assessment from the Therapy Services when determined by the Physiotherapist/Occupational Therapist To manage own caseload including ward management using comprehensive risk assessment/management under the guidance of a qualified Physiotherapist/Occupational Therapist To carry out community visits including home visits and access visits To undertake specific clinical responsibilities or projects supporting the overall delivery of Therapy services.
