About the RoleWe are expanding our innovative Rapid Response & Intermediate Care (RRIC) service across Buckinghamshire. You will work with our Aylesbury team across Intermediate Care and Community Physiotherapy. ResponsibilitiesProvide expert interventions and rehabilitation as part of a dynamic team responding quickly to crises. Help patients stay in their own homes, supporting recovery, early hospital discharge, and end-of-life care. Contribute to triage, assessment, care planning, and case management of patients and carers. Collaborate with multidisciplinary team members including Physiotherapists, Nurses, Paramedics, Pharmacists, and Healthcare Assistants. Support development of clinical pathways and work across Intermediate Care and Physiotherapy pathways.
