For more information, visit: work as directed and under the supervision of the registered nurse/midwife in charge of the ward/department.Undertake delegated individual care needs required for the comfort and well-being of patients, following care plans or Integrated Care Pathways, including personal hygiene and continence care.Contribute to the movement and handling of patients to maximize their physical comfort.Assist patients to eat and drink, documenting all intake and output.Effectively communicate changes in patient condition or any untoward incidents to appropriate team members.Involve patients in decision-making processes and obtain their consent before starting any care, respecting their choices.Support patients' individual rights and personal cultural beliefs.Maintain patient dignity and meet their specific needs at all times.Plan your working day, considering responsibilities and prioritizing workload in consultation with a qualified nurse.Promote health education whenever possible.Document all individual care provided to patients and report any variances to a Registered Nurse.Work in accordance with the Trust's Health Care Support Worker Code of Conduct and ensure clinical competence has been assessed and maintained.Ensure effective communication channels between patients, relatives, staff, and outside agencies as appropriate.This advert closes on Friday, 16 May 2025.#J-18808-Ljbffr
