We have an exciting new post for the Falls Prevention Brighton and Hove team, part-time 24 hours per week. This role involves a variety of duties, including: To identify and carry out specialist assessments of individuals at risk of falls and fractures. To plan and provide care programs tailored to individuals' complex needs across various settings, including clinics, exercise groups, health promotion, and education. To manage a caseload of patients using specialist clinical assessment, diagnostic, and treatment skills for fall risk management. To provide training and education on falls prevention to staff across the local health and social care sectors, including residential and home care services and voluntary organizations. To supervise assistant staff/support workers and undergraduate students, providing training to staff and carers. To develop strong links with the Primary Care Trust, Sussex Ambulance, voluntary and independent sectors, and the local community to deliver an effective service. To adhere to professional standards and guidelines of practice, education, and conduct. Main duties of the jobSupport the Falls and Fracture Prevention Service Manager in monitoring and evaluating the Falls Service across Brighton and Hove. Assist in service evaluation through clinical audits as part of the team's effectiveness plan. Maintain accurate data entry according to Trust policy, including activity/contact data using systems like PIMS/DISCO. Develop links with Fracture Prevention Specialists, Consultant Geriatricians, and the Royal Osteoporosis Society for coordinated service delivery. Coordinate with consultant Geriatricians regarding the diagnostic falls clinic and manage referrals. Perform additional tasks as assigned by the Osteoporosis and Falls Prevention Service Lead. Support the development of the service, including care pathways, policies, and procedures based on the Calderdale Framework. Person SpecificationQualificationsProfessional qualification such as a degree or equivalent in Physiotherapy or Occupational Therapy. Current registration with the Health and Care Professions Council as a Physiotherapist or Occupational Therapist. Evidence of relevant postgraduate courses / CPD and reflective practice, e.g., PSI, OTAGO. ExperienceEvidence of a range of clinical competencies appropriate for the role. Significant experience of supervision provision.
