Job summary Cape Hill Medical Centre has an exciting opportunity for an ANP to join our friendly, hardworking and patient focused practice team. We are looking for a team player with the enthusiasm to make a difference to primary care. We are a training practice, with 4 Clinical partners, 6 salaried GPs, 4 ANP, 2 Practice Nurses and 1 HCA. The entire clinical team is supported by a fully staffed admin and reception team allowing us to provide excellent patient care. We are part of CARITAS PCN with another 2 local Practices with a cohort of additional roles staff support to share and deliver clinical care to ensure the patients see the right clinician for their needs Main duties of the jobAn experienced ANP resource for health care professionals and service users, working in collaboration with other members of our multi-disciplinary team. Assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury. You will hold experience and skills to be able to assess, diagnose, treat and refer patients with a range of chronic diseases and / or specialist support e.g. diabetes, asthma, heart failure, rheumatology. Use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment. An experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice You will demonstrate critical thinking in the clinical decision-making process. Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
