Band 5 Community Health Nurse - Fixed term - 12 mths £34,521 - £41,956 pro rata per annum, inclusive of HCAS JOB SUMMARYWe are seeking dedicated professionals to join our health visiting team, supporting children and families within a defined community. This role focuses on improving health and life outcomes by delivering care packages following assessment by a Health Visitor. As part of the team, you will support Health Visitors and Community Nursery Nurses in delivering theHealthy Child Programme (0-5 years) During your time in this role, you will gainhigh-quality exposure and training , preparing you to apply for aSpecialist Community Public Health Nursing (SCPHN) programmein September 2026, either internally or externally through Bromley Healthcare. If you choose to apply for the SCPHN programme, you must submit a formal application, as this role does not guarantee acceptance. The programme is taught atLevel 7 (Master's level)and applicants should be prepared for the academic rigor involved. The selection process is highly competitive, and candidates will need to attend an additional interview in 2026. As this is a fixed-term contract, if you decidenotto apply for the SCPHN programme, your contract with Bromley Healthcare will end inSeptember 2026 . Internal applicants have the opportunity to apply for this role on a secondment basis . Interviews expected to be held week commencing 16th June 2025 Main duties of the jobThis is afixed-term position for a minimum of 12 months , ideal for individuals with a strong interest in children's public health nursing who aspire to a career in Health Visiting. To support the delivery of the Local Health Visiting offer, including the Healthy Child Programme. Additional responsibilities include:Work as an independent practitioner within a skill-mixed team, supporting staff as needed. Undertake delegated work promptly and efficiently. Identify children and/or families with additional needs, referring to other agencies/teams for support and intervention as appropriate. Provide health advice and signpost children and families to local community resources. Undertake developmental assessments and 6-8 week reviews when trained and competent. Deliver health promotion activities and targeted interventions. Follow safeguarding protocols to ensure children's safety. Carry out early intervention, safeguarding, and multi-disciplinary health referrals. Conduct home visits as part of health objectives. Liaise with GPs and Primary Care Team members as appropriate. Qualifications and experienceRegistered Nurse or Midwife. Willing to undertake further training and CPD. Interest in Public Health Nursing / Community work. Post-graduate experience in Nursing or Midwifery. Commitment to applying for SCPHN within 12 months. Skills and attributesReflective, emotionally intelligent, and supportive practitioner. Effective communicator, both written and verbal. Ability to work independently and prioritize. Team player with collaborative skills. Good IT skills (Microsoft Office). KnowledgeUnderstanding of NMC Code of Conduct. Awareness of NHS pressures related to Public Health. Personal qualitiesPositive, enthusiastic, and flexible.
