Closing Date: Thursday 5th June 2025Job Title: Accommodation Pathways CoordinatorReports to: Youth Services ManagerSalary: Scale 6 - Point 26- 28, £26,229 - £27,982 per annum, pro rataHours of work: Full Time (37.5 hours per week)*Fixed Term Contract for 6 months* Key Responsibilities: To receive and respond to referrals from /Social Workers/Personal advisers and placement officers, in a timely manner. To undertake strengths-based, person centred housing and support needs and risk assessments. To plan responses to presenting needs and circumstances in partnership with young people, parents/carers, other agencies and professionals to prevent homelessness, identify and negotiate the most suitable housing options. To work within the resources provided by Birmingham Children's Trust Commissioning Team or Vulnerable Adults provision. To ensure that placements and services identified through in-depth risk management plans meet the needs of young people referred, considering issues in respect of health, educational need, ability, gender, religion, ethnic origin, culture and linguistic background. To provide professional advice, information, and support to a range of key stakeholders such as Social Workers, Heads of Service, Youth Offending/probation Teams on a range of services/ provisions that are available to meet the housing and support needs of young people. To manage and prioritise own caseload and evaluate each case making decisions on appropriate options, meeting deadlines and referring to other services and outside agencies as appropriate. To represent St Basils through attendance at designated meetings i.e. monthly Support Provider and contract meetings and case reviews meetings regarding young people, as and when required. To ensure the service is accessible to young people, all relevant parties kept abreast of outcomes and follow-up support provided as and when required. To keep up to date with current legislation, regulations and other new developments relating to young people and housing resources etc. To assist in the service development and achievement of key performance indicators in line with contract specifications and returns. To highlight trends, areas for development and gaps in service provision, to Line Manage and assist in developing action plans. To assist in updating policies and procedures. To maintain statistical information for monitoring and evaluation purposes in order to produce reports and statistics for key stakeholders such as Birmingham children's trust as and when required. To liaise with key partners/funders as appropriate. To represent and promote the work of the agency and/or project at relevant external and internal meetings and forums. To deliver training and briefings as and when required. To be involved in the induction of new staff, to supervise and support volunteers, trainees and students. To work closely with other organisations and agencies to provide holistic multiagency responses to housing and support needs i.e. DWP, FTB etc. ROLE EXPERIENCE Working with young people in the following settings/Types of work - Youth Work, Residential or Social Work, Probation, Community Work, Education and Training, Housing. • Experience of working with and delivering a service through payments by outcomes PBO or PBR• Experience of working with young people with multiple complex needs.• Experience of Safeguarding vulnerable children, young people, or adults QUALIFICTAIONS Youth Work, Social Work, or equivalent NVQ 4 Advice & Guidance, Care or equivalent or working towards Department Operations Locations Birmingham
