Job Overview12 month fixed term contract for external applicants or 12 month secondment opportunity for internal applicants.Are you a registered Mental Health Nurse, Social Worker or Occupational Therapist?Our Crisis Resolution Home Team is looking to recruit a Mental Health PractitionerThe team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week on a varying rostered shift pattern. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists Social Workers, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.In the role you will deliver a high-quality clinical service to patients and their families.You will deliver therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.Main duties of the jobMental Health Practitioner – Nurse, OT, Social WorkersWorking in a multi-disciplinary team and in partnership with internal and external agencies, you will;Provide specialist assessment and care planning in conjunction with service users and carers Evaluate treatment plans and undertake home treatment Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues Co-ordinate the work of the TeamWorking for our organisationBy joining Team MPFT, you will be helping your communities and in return for this, we will support you by;Supporting your career development and progression Excellent NHS Pension scheme Generous maternity, paternity and adoption leave Options for flexible working Up to 27 days annual leave (increasing with service up to 33 days) and the opportunity to purchase additional leave Extensive Health and Wellbeing support and resources If you work in our community teams, we pay for your time travelling between patients Lease car if you complete more than 500 business miles per annum, fully insured and maintained (including tyres), mileage paid at lease car rate Salary sacrifice car - fully insured and maintained (including tyres), your gross pay is reduced by the cost of the vehicle before tax, NI and pension deductions are calculated, mileage paid at business rates Salary sacrifice bikes up to £2k Free car parking at all trust sites Free flu vaccinations every year Citizens Advice support linked with a Hardship Fund for one off additional support up to £250 (if the criteria is met)And more.
