For part-time roles, the salary and payments will be pro rata based on contracted hours.ResponsibilitiesThe RoleAs a valued member of our Reading team you'll be taking on responsibilities includingWork in close partnership with A&E and other departments to develop and implement care plans for frequent hospital attenders. Be that first point of contact for Hospital staff in relation to our service users to ensure seamless delivery of care. Providing specialist assessment of patients attending/admitted to hospital with alcohol related issues using AUDIT screening tools and to provide brief interventions and onward referral for patients. Providing advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways. Working closely with our community substance misuse teams to ensure assertive and effective follow up of repeat presentations and discharges from the hospital. Keeping GP's informed of patient’s status and discharge plan/referrals within 5 days of patient’s hospital discharge. Developing specific links with identified specialist acute services. Educate and support carers and contribute to the assessment of carers’ needs.About YouTo thrive in this role, we're looking for someone who hasExcellent understanding of drug and alcohol issues and experience of working within a related field.
