Occupational Therapist - ESD for Stroke B6We have an exciting opportunity for an enthusiastic Occupational Therapist to join our established Stroke Early Supported Discharge Service. We work closely with acute stroke services in our area and our neurology colleagues in the community to provide continuity of care across the stroke pathway. Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity. We support safe discharges home and facilitate rehabilitation therapy in patients' places of residence. We are looking for a qualified occupational therapist with post-graduate experience working with stroke patients, who can manage their own caseload, whilst collaborating with the multidisciplinary team and partner organisations. You will have the opportunity to utilise your occupational therapy skills to the full. As well as conducting assessments and devising rehabilitation programmes aligned to patients' goals, we provide education, emotional support and carer support. The immediate team includes occupational therapists, physiotherapists, a speech and language therapist and therapy assistant practitioners. Job responsibilitiesClinical To be professionally and legally responsible andaccountable for all aspects of your own work including the management ofpatients in your care. To ensure a high standard of clinical care for thepatients under your management, and support assistants to do likewise. To accept clinical responsibility for a designatedcaseload of patients, and to organise this efficiently and effectively withregard to clinical priorities and use of time. To undertake the detailed assessment of patients, someof whom may have complex needs, using investigative and analytical skills. Tocollaboratively work with patients to develop rehabilitation goals. To formulate an individualised treatment plan, usingclinical reasoning skills and utilising a wide range of skills and treatmentoptions. To develop specialised skills in relation tocognition, perception, and upper limb therapy. To provide advice to the MDT regarding the managementof patients requiring therapy input. To provide spontaneous and planned advice, teachingand instruction to relatives, carers, schools, workplace and otherprofessionals, to promote understanding of the aims of Occupational Therapy toensure a consistent approach to patient care. To communicate effectively with patients and carers tomaximise rehabilitation potential and to ensure understanding of their munication skills of persuasion, motivation, explanation and gaininginformed consent will be used with a wide variety of patients. Barriers toeffective communication may include loss of hearing, cognitive difficulties,expressive and receptive aphasia, pain and fear. To assess capacity, gain validconsent and have the ability to work within a legal framework with patients wholack capacity to consent to treatment. To help facilitate patients toself-manage their rehabilitation. To take responsibility for appropriate delegation oftasks. To train, supervise and manageperformance of assistant practitioners, therapy assistants and students, withassistance from more senior members of the team. This will include the use of formalappraisal documentation. To maintain up-to-date knowledge ofthe full stroke care pathway. Professional To maintain own clinical professional development(CPD) by keeping abreast of any new trends and developments and incorporatethem as necessary into your work. To be an active member of the in-service trainingprogramme by the attendance and presentation at staff meetings, tutorials,training sessions, external courses and reflective practice. To communicate effectively and workcollaboratively with medical, nursing and therapy colleagues to ensure deliveryof a co-ordinated multidisciplinary service.This will include case conference and medical reports. To provide teaching/training to other membersof the multidisciplinary team. To participate in the staff appraisal scheme andpersonal development. To undertake the measurement and evaluation of yourwork and current practices through the use of Evidence Based Practice projects,audit and outcome measures. To make recommendations for and assist with change. To demonstrate a sound understanding of ClinicalGovernance and Risk Management and apply to work situation.
