Due to increasing demand, our client is seeking a dedicated and experienced Client Care Manager to join their compassionate and professional team. Role Overview You will be responsible for a portfolio of clients — from newly onboarded to existing service users. Your focus will be on writing and updating care plans, conducting risk assessments, overseeing care quality, and collaborating with families, Care Professionals, and healthcare contacts. Key Responsibilities * Manage a portfolio of clients in accordance with company policies and CQC standards * Work alongside the Service Manager and Head of People to ensure clients receive well-matched, trained Care Professionals * Coordinate safe hospital discharges, engaging with discharge teams and community services * Liaise with GPs, pharmacies, district nurses, social workers, and other professionals * Confidently refer clients to relevant community services and external support * Champion high standards of person-centred care * Offer professional support and reassurance to family members Person Specification * Minimum 2 years’ experience in a Care Management role * Proven experience writing personalised care plans * Strong knowledge of CQC regulations and KLOEs * Experience with care/people management systems * Minimum 5 GCSEs A–C including English and Maths * Must be based locally to New Malden * Excellent communication, time management, and organisational skills * Professional, caring and positive attitude
