Job Opportunity: Band 4 Pharmacy Technician An opportunity has arisen for a Band 4 Pharmacy Technician to join our Integrated Community Services (ICSD) Pharmacy team. The ICSD Pharmacy team plays a key role in supporting patients following hospital admission and preventing avoidable admissions during periods of acute illness.The role involves working under supervision as part of a multidisciplinary team, including GPs, Consultants, Nurses, Pharmacy staff, Support Workers, Therapists, independent providers, and colleagues within the Local Authority. We also collaborate with independent care providers, offering training on medication management.The service aims to deliver safe, effective, and cost-efficient prescribing through medicines optimisation, waste minimisation, reducing polypharmacy, and addressing safeguarding issues related to medication. We work across the healthcare system with care homes, community services, CCGs, Bolton NHS Foundation Trust, social care, community pharmacy, families, and carers to optimise medicines use for residents and individuals in their own homes. Our goal is to support individuals to achieve appropriate independence with their medicines and to assist healthcare professionals in medicines management.Responsibilities include working with the ICSD Pharmacy team to improve medicines management across community services such as Neighbourhood teams, Admission Avoidance, Home First, Virtual Wards, Home Support Reablement, and bed-based Intermediate Care.Key ResponsibilitiesSupport the safe and cost-effective delivery of pharmaceutical services to patients in bed-based services and domiciliary care where appropriate.Assist with clinical pharmacy services, including medicines reconciliation and administration support.Our ValuesVision:We aim to deliver excellent health and care for future generations, working with partners to ensure sustainability.
