About the Role The Prevention Officer is an exciting position that has been created as part of the Face-to-Face Prevention project within the Adults Transformation Programme. You will play a key role in promoting independence and well-being among adults in the community. We have the opportunity to recruit to 2 of these new Prevention Officer posts. The Prevention Officer will provide support that focuses on prevention, supporting people through crisis and helping them to become independent, utilising their own strengths and support networks in their area. Workers will have a proactive approach to preventing an individual's health and social care issues before they escalate. You will be required to collaborate with the individuals personal support networks, health professionals and community partners to help deliver effective prevention strategies. About You The ideal candidate will have: You will need excellent communication and time management skills and must be highly motivated, enthusiastic and committed. You will be able to demonstrate a flexible approach and a strong willingness to accept and adapt to change. You will be responsible for assessing vulnerable adults in their own homes and the nature of the work requires significant resilience and an ability to focus on outcomes. THE ESSENTIAL CRITERIA Relevant experience in Adult Social Care or a related field. Strong assessment and decision-making skills. Excellent communication and interpersonal skills and an ability to build effective working relationships at all levels Ability to work independently and as part of a multidisciplinary team A commitment to promoting independence and dignity in care delivery A DBS enhanced check for a regulated activity is required for this post.
