We support patient flow through rapid response, admission prevention, and facilitation of hospital discharges with a Home First approach. You will also be promoting falls management, helping people to live well with long-term conditions, palliative conditions, and supporting patients at the end of their lives. You will gain further experience in areas including neurology, musculoskeletal, orthopaedics, respiratory, rheumatology, frailty, and mental health including dementia. Main duties of the jobYou will have an opportunity to make a positive difference to your patients' lives and develop skills in triage and management of deteriorating patients. You will work closely with the wider multidisciplinary team to enhance your knowledge and will be supported by clinical supervision, peer support, and regular continuing professional development. You will be responsible for your own caseload, delegating to others to optimise outcomes, and delivering holistic rehabilitation made meaningful by taking place in local communities. You will develop leadership skills by supporting others including therapy assistants and students, and contribute to service improvement projects across the Trust and wider partners. The service operates over 7 days from 8 am to 6 pm, and you will be required to lone work. We promote a positive work/life balance and utilize flexible working to accommodate your needs. We have posts in the North and South hub areas, with an understanding that this is a citywide service and there may be a requirement for cross-cover working.
