To assess, triage, diagnose and treat patients face to face accordingly, performing specialist assessment of their needs and make referrals as necessary. To request diagnostic investigations and interpret within agreed protocols e.g x-ray, blood tests and scans. To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. As an independent prescriber, to review and prescribe medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols.About UsSouth Hornchurch Medical Practice has approximately 6000 patients and we are located in a health centre with parking on site. You will be working alongside 1 Lead GP and 3 female locum doctors, an Advanced Clinical Prescriber, a Practice Nurse and HCA with additional PCN Staff including a Pharmacist, Physiotherapist, Dietician and a Social Prescriber. Evidence of appropriate continuing professional development activity to maintain up -to date knowledge and ongoing competence in all aspects of the ANP role, including long term condition management. Evidence of working autonomously and as part of a team.
