Job Description

Part-time role for 25-30 hours per week. This advert will close early if sufficient applicants are received. The Clinical Services Care Coordinator is primarily responsible for supporting the Clinical Services Team with regards to delivery of services offered at the practice. The focus of the team is to ensure high-quality care for patients through a well-coordinated administrative system, which offers a holistic approach to improve patient outcomes and experience. The role includes providing administrative support to help deliver GMC and non-GMC work, including the Quality Outcome Framework (QOF), Enhanced Services, AQP Contracts, and other schemes as required. Liaising with and supporting the Clinical Team, practice staff, contractors associated with the practice, and third parties. Helping to improve systems and processes, including utilizing software/technology to enhance patient care and outcomes. Main duties of the jobProvide administrative support to the Clinical Services Team regarding GMS and Non-GMS work, including QOF, Enhanced Services, AQP Contracts, and other schemes. Liaise with and support the clinical team, practice staff, associated contractors, and third parties as necessary. Effective use of IT/software to assist in managing the practice population, including EMIS, Ardens, and iPlato. Utilize EMIS searches and reports for chronic disease monitoring, audits, and other monitoring as required. Assist with mail merges and text messaging campaigns for chronic disease management, vaccinations, health promotion, or patient feedback. Support the Coding Team with filing documents and basic coding as required. Attend internal and external meetings as appropriate. Keep up-to-date with mandatory and role-specific training, and attend relevant courses and events. Assist in solving day-to-day IT, systems, and workload issues. Maintain familiarity with organizational policies and procedures. Undertake other duties appropriate to the role.
