Are you a passionate and experienced Practice Nurse with a special interest or qualification in diabetes care ? About the Role: Youll lead on diabetes management and patient education, working closely with our multidisciplinary team to deliver high-quality, patient-centred care. This is a fantastic opportunity to play a pivotal role in developing and improving chronic disease management pathways in a supportive and innovative environment.
