Operating autonomously and collaboratively, you will assess, diagnose, treat, and manage patients presenting with a wide range of acute and chronic conditions. Your expertise will contribute to the development and implementation of evidence-based clinical guidelines, ensuring optimal patient outcomes and service delivery.Key Responsibilities: Clinical Assessment & Diagnosis: Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.Patient Management: Prescribe medications (as an independent prescriber), initiate and manage treatment regimens, and provide follow-up care.Chronic Disease Management: Lead clinics for long-term conditions such as diabetes, hypertension, and asthma, implementing personalized care plans.Clinical Leadership: Provide mentorship and support to junior nursing staff, fostering a culture of continuous professional development.Quality Improvement: Participate in clinical audits, research initiatives, and service evaluations to enhance care quality and patient safety.Collaboration: Work closely with GPs, consultants, and other healthcare professionals to deliver integrated care pathways.Health Promotion: Develop and deliver health education and promotion programs to patients and the wider community.Record Keeping: Maintain accurate and up-to-date patient records in accordance with legal and professional standards.Essential Qualifications & Experience: Registered Nurse with a valid NMC PIN.Masters degree in Advanced Clinical Practice (or working towards).Independent Prescriber status (V300).Minimum of 2 years' post-registration experience in a relevant clinical setting.Demonstrated competence in advanced clinical assessment and decision-making.Evidence of continuing professional development and reflective practiceBenefits: Competitive salary.Annual leave entitlementAccess to ongoing professional development and training opportunities.Supportive working environment with a focus on work-life balance.Opportunities for career progression within the organisationPlease apply here!TPBN1_UKTJ
