This is an opportunity to further develop your career working across both Health and Social care.You will work as a member of the Community Occupational Therapy Team, promoting independent living and well being through individualised assessments which may lead to the provision of specialist equipment or environmental adaptations to the home.The Community Occupational Therapy team is based at the Link Centre and part of the Integrated Therapy Hub, which comprises of Community Neuro Rehab, Falls, Stroke, Dietetics, Speech and Language Therapy, Macmillan AHP's, Community Physio.Main duties of the jobWorking with both adults and children experiencing difficulties managing everyday activities due to disability/illness within their home environment this is a varied and diverse role.You will actively liaise with other services in the community, primary and secondary care to ensure the necessary support is available.Your will provide support/supervision/training to peers and junior Occupational Therapy staff as required.Further details can be found within the job description and person specificationThere may be a requirement to work flexibly which may include evenings and weekends.Key Responsibilities Of The Post:Assess the functional abilities of people whose needs can be complex, changing and vary across the caseload.Provide professional guidance and support for Technical Instructor III's/OT Assistants.To assess and treat patients and develop, implement, evaluate and modify individualised treatment program.To carry a caseload of patients that present with acute or chronic conditionsTo participate in appropriate meetings e.g. MDT'sAssess, plan, prioritise and make recommendations with regard to equipment, adaptations etc. as appropriate to the client group.
