Do you want to help improve the lives of children and young people who care for a family member? n-Compass is looking for a dedicated Young Carers Practitioner to join our committed and dynamic Warrington team. About the Role As a Young Carers Practitioner, you will play a vital role in supporting children and young people who care for a loved one. You will: Contact new Young Carers and welcome them to our service. Gain an understanding of each Young Carers unique caring role and develop tailored support plans to reduce the impact of their responsibilities. Make appropriate referrals for statutory Young Carers Assessments. Provide ongoing, time-limited one-to-one support, regularly reviewing and adapting support plans as needed. Build positive relationships and connect Young Carers and their families with a wide range of community resources and support services. Help Young Carers understand the condition of the person they care for, and provide relevant health and social care information. Participate in multi-agency meetings, including Team Around the Family, Child Protection Conferences, and Child-in-Need reviews. Create and lead peer support opportunities such as groups, activities, and trips including during evenings, weekends, and school holidays. Develop and deliver training sessions to help Young Carers build skills for their caring roles and future aspirations. Maintain accurate, confidential records in line with data protection requirements. Use digital platforms and social media to offer information, advice, and support. Actively identify new Young Carers through community outreach and engagement events. Promote and safeguard the welfare of all children, young people, and vulnerable adults you work with. About You You will be an excellent communicator with strong organisational skills and a real commitment to empowering young people. Youll be comfortable working both independently and as part of a team, and have experience working in a similar role supporting children, families, or vulnerable individuals. A working knowledge of safeguarding procedures and the ability to build trust with young people and families is essential.
