Be part of the transformation!Job responsibilities PurposeTo work as a highly capable practitioner assessing and managing complex, high-risk cases within a locality or district team. To recommend person-centred support and outcomes for individuals in Cumbria with social care needs, and advise on developing and implementing care/support programs for individuals, groups, and communities. The role involves significant autonomy, managing complex cases, and making decisions based on best practices, risk management, and empowerment approaches. The practitioner will have specialist knowledge, exercise sound judgment, and offer innovative, solution-focused approaches, using reablement to maximize individual potential.
