Over the last few years, we have strived toimprove services for patients and staff, developing and implementing ambulatoryECG, the management of post-bariatric services in the community, decisionmaking and referral support software for GPs, a clinical pharmacy service forthirteen member practices, and post discharge clinics that reduce theadministration burden on surgeries. We are looking for a dedicated and dynamic practicenurse with a strong team ethos to join our team to further enhance our nursingcomplement. The candidate will also have the potential to develop their skills,lead on clinical areas and support teaching and training if desired. Weuse EMISWEB as our clinical information system. Main duties of the jobThe focus of thisrole will be the development and implementation of processes and care for theeffective management of patients with long term conditions, utilising evidencebased practice and promoting self-care for these patients. The post holder willhave a significant role to play in leading further quality improvementprocesses with the rest of the clinical team. There will also bethe requirement for the post holder to the deliver general practice nursing andpreventative health care to the practice population. As an autonomouspractitioner the nurse is responsible for the care they deliver, demonstratingcritical thinking and skills in clinical decision-making for the optimummanagement of patients. They will work collaboratively within the practice teamto meet the needs of patients, support the development and delivery of policyand implementation of procedures. They will lead by example with particular reference to promotingexcellence in preventative healthcare, communication with patients, equalityand diversity and patient confidentiality. Recordsdata in patient records systems promptly and accurately and to agreedstandards. Ensuresappropriate use of read codes. Participatesin the auditing of practice activity as required. Responsiblefor health and safety of themselves and others and actively co-operates with Health and Safety Policies and procedures. Processesand actions incoming patient and hospital correspondence. Attendsin-house meetings, significant event audit meetings and other meetings asdeemed necessary. Ensuresthat the practice complaints system is adhered to at all times. Awarenessand compliance with all relevant guidelines e.g. data protection,confidentiality and health and safety. Maintaincurrent CPR and anaphylaxis certification and child/adult safeguarding trainingin line with local policies. Training &Development Fullyparticipates in training and development Engagesin a programme of ongoing support and feedback to maximise the benefit of thetraining and development plan. Confidentiality In the course of seekingtreatment, patients entrust us with, or allow us to gather, sensitiveinformation in relation to their health. They do so in confidence and have theright to expect that staff will respect their privacy and act appropriately. Information relating to patients,carers, colleagues, other healthcare workers or the business of the Practicemay only be divulged to authorised persons in accordance with the practice policiesand procedures relating to confidentiality, and the protection of personal andsensitive data. Health & Safety The post-holder will assist inpromoting and maintaining their own and others health, safety and security asdefined in the Practice Health & Safety Policy to include: Identifyingthe risk involved in work activities and undertaking such activities in a waythat manages those risks. Usingappropriate infection control procedures particularly those relating toneedlestick injuries, maintaining work areas in a tidy and safe way and freefrom hazards. Ensuringthat all accidents or dangerous accidents are reported and investigated, andfollow up action taken where necessary. Maintaintraining in line with local policies.
