Slaithwaite Health Centre are looking to recruit an enthusiastic, motivated, experienced Practice Nurse to join the team As a member of our Nursing Team, the post holder will be responsible for the delivery of a defined range of patient care, including basic treatment room care, chronic disease management, and cervical smear taking. The successful applicant must be a qualified registered nurse. Your personal and professional development will be fully supported by the management team. You will be working within a caring and dedicated team of GPs, Nurses and Reception team who care very deeply about the patients they look after. We are looking for a Practice Nurse who can work 8-6 Thursday and Friday Job responsibilitiesThe following are the coreresponsibilities of the Practice Nurse. There may be, on occasion, a requirementto carry out other tasks. This will be dependent upon factors such as workloadand staffing levels: To develop, implement and embed aneffective care management programme for the frail and housebound patientsregistered at the practice To assess the needs of patientsensuring the provision of healthcare is appropriate, incorporatingevidence-based practice To develop, implement and embedhealth promotion and wellbeing programmes To implement and evaluateindividual treatment plans for chronic disease patients that promote health andwellbeing To identify, manage and supportpatients at risk of developing long-term conditions, preventing adverse effectsto patients health As required, to provide routinenursing care to patients in accordance with clinical based evidence, NICE andthe NSF To co-ordinate the provision oftravel medicine services To request pathology services andprocess pathology results as required To oversee the provision ofchronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required To maintain accurate clinicalrecords in conjunction with extant legislation To ensure read codes are usedeffectively by all members of the nursing team To ensure the appropriatemaintenance and use of chronic disease registers To develop, implement, embed andparticipate in well woman clinics To develop, implement, embed andparticipate in well man clinics To ensure compliance with thepractice chaperone policy To assist GPs with minor surgerywhen required To prioritise health issues andintervene appropriately To support the team in dealingwith clinical emergencies To implement vaccinationprogrammes for adults and children Cervical Screening Family Planning Person SpecificationQualificationsRegistered Nurse (Nursing and Midwifery Council) Post graduate diploma or degree (Chronic Disease Management) Nurse prescriber Family planning qualification ExperienceExperience of working in a primary care environment Experience of chronic disease management Experience of working autonomously Experience of infection prevention and control measures Experience of managing elderly and housebound patients Experience of quality initiatives, i.e., benchmarking Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.#J-18808-Ljbffr
