Ratby Medical Centre is part of the Bosworth PCN. With this position, we will be continuing to grow are Pharmacy Technician team, with the aim of it being a full comprehensive team of 6 Pharmacy Technicians by the end of this calendar year. We are seeking somebody that is enthusiastic and forward-thinking who wants to support us in this exciting expansion of our Pharmacy team. The successful applicant will play an integral role in complementing our clinical pharmacists within the practice but also be engaged with the other Pharmacy Technicians as we continue to expand the scope of work for the team, providing you with additional challenges and the opportunity to expand your skills and knowledge. Main duties of the jobThe post holder will: Support the role of PCN Clinical Pharmacist within the practice, in delivery of clinical pharmacy service Perform medicines improvement work to maximise safe, cost effective best practice in prescribing under the supervision of a GP or the Clinical Pharmacist Improve the use of medicines and help patients with medication adherence Assist with medication safety monitoring systems (e.g. high-risk drugs) Liaise with clinicians and administrative staff to resolve medicine-related queries. Monitor practice prescribing and adherence to practice, local and national prescribing policies Improve services and quality through mechanisms such as audits To support with the requirements as outlines in the PCN DES. At this time, we have over 10 different PCN staff grades, directly employed with ourselves, with further sub-contracts in place to cover other clinical grades. We also hold the contract for The Centre Surgery, and directly employ the staff working within the practice. There are a further 11 staff based in the Head Office, as well as have a substantial list of bank staff whom support us with the Enhanced Access service. Job responsibilitiesJob Summary: We are seeking an enthusiastic and forward-thinking pharmacytechnician to join our like-minded PCN. We endeavour to find the best pharmacytalent to join our team. The successful pharmacy technician candidate will playan important role in complementing our clinical pharmacists and other membersof the PCN multi-disciplinary team. This is an evolving job role with hugescope for development as we expand our PCN team further over the years. It is a requirement of the role to be registered with the GeneralPharmaceutical Council as a Pharmacy Technician. We are seeking a pharmacytechnician preferably with previous experience of working in primary care.The Pharmacy Technician role is a new development within our PCN and we expectthe successful candidate to contribute to the delivery of a comprehensiveprimary care medicines management system acting on current best practice /service change requests from our clinical commissioning group. The PharmacyTechnician will be required to undertake activities predominantly at a practicelevel, but will need to be flexible to work to the wider demands of thenetwork. For example, the role will involve liaising with community pharmacyteams to highlight and manage problems identified by community pharmacy, suchas medicines waste.An understanding of primary care operations and use of SystmOne would be anadvantage but is not essential. Primary Duties and Areas of Responsibility: Operational Support the Clinical Pharmacists in Structured Medication Reviews(e.g. organise necessary monitoring tests before SMR and identification of appropriate patients) Undertake patient-supporting roles to ensure effective medicinesuse through shared decision-making Support medicine reconciliation of patients whose care istransferred back into primary care in a timely and effective manner liaisingwith patients and other providers to ensure patients receive appropriatemedication on discharge Liaisewith PCN dispensaries and community pharmacies to aid effective medicinesreconciliation Carry out medicines optimisation tasks including effectivemedicine administration, supporting medication reviews and medicinesreconciliation post-discharge and other transfers of care Assist with the development and implementation of systems toensure safe and effective monitoring of high-risk medication Support initiatives for antimicrobial stewardship to reduceinappropriate antibiotic prescribing Assist Clinical Pharmacists with any searches, data collection,medicine safety tasks Work with pharmacy team to develop safe, effective and efficientsystems for repeat prescribing Increase uptake ofElectronic Repeat Dispensing Support the PCN to deliver on QIPP agenda, QOF and locallycommissioned enhanced services Review and analyse prescribing data across the network usingavailable tools producing reports and feedback for practices Undertake, participate and share outcomes of clinical audits tofacilitate improvement Implement changes to medication that result from CAS/MHRA alerts,product withdrawal and other local/national guidance Collaborating Working Relationships Work collaboratively with the PCN team Develop relationships and work closely with community, hospitaland MMT pharmacy colleagues and the wider health system Liaise with other stakeholders for the collective benefit ofpatients Participate in practice meetings, patient participation groups andother meetings to improve engagement with the role of pharmacy technician Practice Targets Support practices in achieving QOF To be aware of the prescribing budget and identify and actionareas for cost improvement Work with PCN team to ensure the practice is compliant with CQCstandards where medicines are involved PCN responsibilities: Identify,report and or process any issues of concern relating to safeguarding andquality of care arising from working with patients and referring onwards asappropriate. Participatein the induction and training of new members of staff and contribute to PCNdevelopment. Work with your line managerto undertake continual personal and professional development, taking an activepart in reviewing and developing the roles and responsibilities.
