FDIDT are an in-reachmultidisciplinary team made up of AHPs, Nurses and Assistant Practitioners.This Nottingham CityCare Partnership service provides innovative, successfuland patient focussed outcomes for those that present in the emergency and urgentpreadmission areas at Nottingham University Hospitals QMC site. Yourrole within the busy and dynamic team will be to provide a comprehensiveholistic assessment of need with the aim of preventing avoidable hospitaladmissions. The successful candidate will beable to work within the MDT as anautonomous practitioner, undertake comprehensive assessments, and complete professionspecific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complexphysical and mental health needs.
