Mental Health Nurse - Band 6/7 equivalentCome and join the largest primary care Mental Health and Wellbeing team in Exeter! As a PCN we are currently working towards some exciting new projects to forge ahead with transformation in same day care, putting patients at the centre of everything we do. As part of this transformative approach to care we are looking to recruit a MH Nurse to support the team working as part of a larger multi-disciplinary team including 2 MH nurses, wellbeing coaches, social prescriber. The role will support existing services and also help develop a new and exciting pathway to supporting low risk patients to access care and services available on the day and to support with follow up routine appointments. This would be an excellent next step for anyone looking to develop their skills within primary care, looking to step into a lead role. Main duties of the jobYou will manage a caseload of clients with complex needs, using evidence based/client centred principles to triage, assess, plan, implement and evaluate interventions. You will provide leadership for students and/or junior staff, through supervision and appraisal. You will participate in the planning, development and evaluation of MH and wellbeing services and work with Health and Wellbeing Coaches, Social Prescribers and other professionals as part of a wider MDT team. In addition you will help to design and inform the same day service team and the contribution that can be made by the PCN MH and Wellbeing Team, working alongside clinical pharmacists and physiotherapist to provide a truly integrated Multi disciplinary team to provide same day support for patients. In addition you will manage soem follow up routine work in tandem with our GP teams and exsisting DPT supports services. About usExeter City PCN support 38000 patients across 6 practices. We already have a well-established Mental Health and Wellbeing Team who work in partnership with our primary care clinicians to provide more services and support closer to patients. Sadly our brilliant MH OT lead is moving on to other opportunities, so we are looking to appoint a proactive MH Nurse to take on the role. The team consists of 2 health and wellbeing coaches, a 2 MH nurses, a young persons MH support worker, social prescriber and brief assessment and intervention worker. Over the past three years the team has developed and grown into the role of providing interventions, coaching and support to primary care patients. They have been very successful in building strong links with Voluntary and Charitable organisations making excellent use of community contacts to support patients. Their innovative approach has help shape and develop services across the PCN such as our Womens Menopause Service, Healthy Movement and Young Peoples Services to engage with support. The team are brimming with ideas and we want to help them achieve them. Job responsibilitiesSummary: Exeter CityPCN (ECPCN) has developed a same day service to support our patients and GPsin providing care for med- low risk patients who present to the practice for onthe day support and advice alongside some routine follow up work. The postholder will work as part of a multi-disciplinary team and within their clinicalcompetencies to provide expertise in mental health management using theirclinical skills to manage patients safely mainly over the telephone with some in person support, and prescribeaccordingly if able and required. This role ispivotal to improving the quality of mental health care for patients across thePCN who contact the surgeries with on the day queries and requests. You willalso be able to support and signpost or refer patients to other appropriateservices or colleagues within the well-established Mental Health and WellbeingTeam. The team comprises of 2 Health Coaches, a Social Prescriber, Assessmentand Brief Intervention Worker (ABI), Mental Health Nurse Prescriber. The postholder role is part of DEVON ICB and Exeter City PCN joint pilot to evaluatethe role of non-GP interventions in increasing capacity in general practice.The post holder may be asked to contribute to data collections and supportreview of the service. Scope and Purpose of the role The post holder will be responsiblefor the ongoing development and delivery of these processes within the PCN, including: Working with the PCN Same Day ServiceTeam to provide support to patients with urgent mental health needs, queries orconcerns. This includes providing telephone and online support as well as occasional in person support.. Supporting patients to make best useof alternative services such as community based voluntary services as well asformal NHS support pathways. Working with the SDS Team to ensurethat clinically med - low risk patients feel listened to and can access advicefor mental health concerns and brief assessments for urgent interventions. Assessment of risk and detailed actions forcolleagues to review and follow up. Work within own professional boundaries and supportteam members to provide a holistic service to patients, and ensuring any issuesidentified during the review are acted upon in a timely fashion. Using specialist knowledge ofbarriers, both real and perceived, in accessing care, adherence to medication,multi-morbidity, frailty and communication skills across a range of physicaland cognitive limitations, to formulate solutions for individual patientsissues, and support patients to make best use of the support available to them. KEY RESPONSIBILITIES: Provide comprehensive assessment, care andsafety planning for patients referred through the PCN practices for on the daysupport. Provide appropriate prescribing withinscope of own practice if appropriate. Provide purposeful interventions andsignposting that promotes recovery and enables patients to self-selectappropriate pathways available to them. Ensure accurate and timely clinicaldocumentation in line with your professional Code and PCN policy. Develop and maintain specialist clinicalskills and expertise in the assessment and treatment of people with mentalhealth needs. Offer advice on management of patients withmental health issues to other health care professionals, advocating for bestpractice in line with PCN and national guidance. Assess, investigate, treat, or referpatients to other services or colleagues within guidelines and the scope ofpractice. Ensure that patients requiring furtherassessment and help are referred on appropriately. Identify and implement effective joint careplanning for people who repeatedly contact the service for assessment. Arrange admission to the mental healthservices in conjunction with the patient's own GP where appropriate. Liaise effectively with staff in localsystems and maintain accurate patient records that provide assessment reportsto GPs and other allied agencies. To provide advice and monitor the use andprovision of the Mental Health/Capacity Act (1983/2005) and other relevantlegislation. To provide clinical advice/expertise healtheducation, consultation and staff support to other health professionals. Have knowledge of how to access appropriateservices and develop effective networks and channels of communication. To actively participate in the achievementof clinical outcome measures and accreditations for the service Managerial: Provide clinical supervision for juniorteam members (wellbeing coaches). Support the Clinical Lead in ensuring theeffective provision of care, service developments of the Same Day Service Team Ensure that referral, assessment, andworkload allocation happens in a timely, flexible and responsive manner. Participate in team meetings, initiatingand discussing new methods/models of care using QI methodology and contribute tothe Devon ICB and Exeter City PCN joint pilot. Participate in the multi-disciplinary andmulti-agency decision-making process through team meetings and team buildingwhen required. Undertake duties as assigned by seniorstaff to ensure that all agreed patient quality, safety and operationalperformance outcomes are delivered within the area of responsibility. Provide specialist advice, support, andconsultation to staff within the team, outside teams and other agencies. The post holder will support theintroduction of outcome measures, to measure patient experience. Contribute towards team education anddevelopment providing opportunities for exchange of ideas, experience,techniques and interventions. A holistic approach to care is requiredthat encompasses physical health, mental health and social care. Take part in service development andreview. Responsibility for Patient/Client Care: Develop and maintain effectiveindividualised care by ensuring the following areas are proactively undertakenat all times: Triage and assessment of client mental,physical and social care needs. Planning, implementation and evaluation ofclinical interventions Clear, accurate and concise documentationof individualised patient care and reflect the changing needs of the patients. Ensure risk assessment and management is anintegral part of everyday practice. All work will be undertaken within thepost holders clinical competence, ensuring that patients are referred to theappropriate clinician as necessary. Workingwith the PCN Same Day Service Team, PCN Teams, GPs and practice teams tofurther develop and participate in the service to all patients within the PCN. Effectively communicating integrated patient centred-care throughappropriate working with the wider health and social care team. Liaising with hospital and community providers wherespecialist expertise is needed above that provided by our PCN teams. Signposting patients and promotingself-care as appropriate. Supervision and Training An Induction programme will beprovided. Monthly meetings with the PCN ClinicalDirector will be provided for more formal supervision. Day-to-day clinical supervision: will alwayshave access to a GP where appropriate in each practice. Clinical supervision provision to be monitoredby PCN Clinical Director. Quality: The post-holder will: Alert other team members to issues of qualityand risk, reporting Significant Events as they arise and participating in the reviewand learning of incidents. Assess own performance and take accountabilityfor own actions, either directly or under supervision. Contribute to the effectiveness of the team byreflecting on own and team activities and making suggestions on ways to improveand enhance the teams performance. Work effectively with individuals in otheragencies to meet patients needs. Effectively manage own time, workload andresources. Person SpecificationExperienceMinimum of 2 years post qualification experience. Experience of working as part of a multi-disciplinary team Experience of remote working and safely managing patient risk over the telephone Ability to work independently in similar role Experience of working in primary care or attached to a primary care team through secondment or training Experience of Mental Health Urgent Care and Assessment Experience of local services in the Devon area. Skills and KnowledgeIn depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare. Ability to review a patient remotely, using clinical skills to ascertain any red flags and provide appropriate advice and/ or treatment. Working within own professional boundaries. Demonstrable ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients, carers, clinicians and administrative staff).
