I am currently recruiting for a Prescribing Pharmacist to work at a GP surgery based in High Wycombe on a 12-month fixed term contract. You must have Primary Care experience and ideally have some background in Asthma, Hypertension or Diabetes clinics. Duties will include: · Contributing to the improvement of health outcomes in long-term conditions · Providing specialist knowledge and advice on pharmaceutical matters · Reviewing patients’ acute medicines requests · Completing discharge and clinic letters · Drawing up structured medication reviews as PCN Pharmacist · Advising and supporting patients with self-limiting conditions ·A practicing Pharmacist with prior experience working as a Clinical Pharmacist, PCN Pharmacist or Practice Pharmacist focussed on delivering excellent clinical care. · Supporting practice achievement of key Impact and Investment Fund - PCN Pharmacist and QOF targets as required GP Pharmacist or Practice Pharmacist This is a 12-month fixed term contract, working 34 hours per week, Monday to Friday.
