This role focuses on supporting vulnerable adults through functional assessments, equipment provision, and major adaptations to promote safety and independence.Key Responsibilities:Manage a complex caseload of community clientsComplete manual handling & equipment risk assessmentsPrescribe and manage major adaptations (e.g., ramps, stairlifts, level access showers)Support hospital discharge and preventionLiaise with housing, DFG, and contractorsProvide clinical input to reports, panels, and safeguarding casesOffer peer support and supervision✅ Requirements:HCPC registered OT with experience in local authority settingsKnowledge of equipment, adaptations & relevant legislationManual handling and risk assessment expertiseEnhanced DBS (we can arrange this)
