Qualified Social Worker, Hospital Discharge to Assess TeamPay rate, £30 per hour Vitalis are recruiting for a Qualified Social Worker to work in a Hospital Discharge to Assess team in Manchester City Council.Social worker required for the hospital discharge to assess South team (Wythenshawe area of Manchester) to support with care act assessments and timely reviews of citizens being discharged from hospital. Experience and knowledge in completing care act assessments,Mental Capacity assessments, best interest decisions and safeguarding is essential. Social worker will support with helping complete timely reviews within a couple of weeks after discharge, reviewing the packages of care that has been put in place by the hospital team and assessing for long term support before transferring to our communityteams.
