This role is integral to delivering person-centred, outcome-focused services that emphasise independenceand recovery. The successful candidate will work collaboratively with health professionals and agencies to ensure safe and timely discharges from hospital and community-based step-down beds. Day-to-day of the role:Deliver a professional Occupational Therapy / Reablement Service, managing a caseload of complex, high-risk cases. Conduct comprehensive, strength-based assessments to create personalised interventions in line with national legislation and local policies. Assess and provide equipment and adaptations to maximise independence and optimise the quality of life for individuals and their carers. Work collaboratively with other professionals to prevent hospital admissions and facilitate timely discharges.
