The experienced recruit will operate within an integrated, multi-disciplinary team and will mainly undertake commissioning and care purchasing tasks for service users, working within the framework of the Mental Capacity Act 2005. Main duties of the jobThe successful candidate will also complete assessments for clients and carers and at times also provide care coordination in line with the Trust Care Programme Approach (CPA) guidance and policies. You will be allocated care coordinator/lead professional for a caseload of both organic & functional clients.
