You will have responsibility for developing relationships with all stakeholders to ensure a seamless a pproach to the care of patients with leg ulcers, working closely with district nurses and primary care services. You will have a responsibility to educate colleagues and train staff in the implementation of integrated pathways of care and delivering on training packages and supervision/mentoring . To provide expert knowledge and advice to other health professionals to ensure patients with leg ulcers have appropriate first line treatment and only referred to the Leg Ulcer service when necessary. To work collaboratively with the DGFT vascular team to ensure a seamless transition of care with exemplary communication skills to be able to expedite patients for assessment to prevent any further deterioration. To support district nurses Promote admission avoidance and early discharge by effective liaison with primary care and the Vascular Team in DGFT.
