Job Role OverviewTitle -Assistant Practitioner Band -4 (£26,530 - £29,114) Hours -22.5 per week Contract -Permanent Base -Mill Hill Health CentreResponsibilities Locala Community Rehabilitation Service provides comprehensive care for adults in community settings. An exciting opportunity has arisen for an Assistant Practitioner to join our team. The post is based in the Orthopedic and Frailty Team in south Kirklees. The team provides rehabilitation to patients with orthopedic conditions following elective joint surgery, fractured neck of femur, frailty, and falls. Colleagues need to be flexible regarding service needs, and there is an expectation that you can travel across Kirklees. The service provides high-quality care to patients in their own homes, community rehabilitation facilities, and care homes. This role requires a motivated and dynamic individual to work within this established service. It is essential that you have experience working as part of a multidisciplinary team, treating patients with complex disabilities, orthopedic conditions, and falls, and experience working in a rehabilitation setting. Good communication skills and flexibility are also essential. A formal Assistant Practitioner qualification is desirable, but willingness to work towards this is essential. We would consider a development role for a Band 3 rehabilitation assistant to progress knowledge and skills through completion of a competency framework. Suitable applicants should have experience working with patients with frailty and orthopedic conditions, some community experience, and have completed core rehab assistant competencies.
