You will be the sole Occupational therapist supported by an experienced Occupational therapy assistant covering East Devon. Be part of a team that valuesregular peer support, supervision, appraisal, and with a strong commitment to learning and development, including access to regular CPD sessions. In this role, you will: Use yourcore occupational therapy skillsto assess the needs of adults with a wide range of disabilities and health conditions. Enable individuals to maximise their independence and potential, while increasing safety and reducing risk to themselves and their carers. Hold and manage a caseload including lone working, providing a person-centred service through clinical assessment within the context of local government policy and legislation. Deliver interventions guided by the Care Act 2014 eligibility criteria and a strengths-based approach. These may include signposting or advice, provision of equipment, minor and major adaptations, support with rehousing, recommendations for care packages, or referrals to other teams. Screen complex referrals, support team leaders through joint visits and peer supervision, and contribute to the development and delivery of updated training for our team of support workers. Act as an autonomous practitioner working across a different community settings . We work closely withNHS colleagues, other adult social care teams, housing providers, and community servicesto deliver timely, person-centred interventions. Our assessments are carried outface-to-face in people’s homes, as well asvirtually or by telephone, depending on the needs and preferences of the individual.
