Community Children's Hospital at Home NurseThe Community Children's Nursing Service is seeking to appoint an enthusiastic and highly motivated individual to contribute to the established Children's Hospital at Home service.The Children's Hospital at Home service is an exciting, dynamic, ever developing team that was set up in 2021 and now offers a number of clinical pathways in which to support children in the community including: Bronchiolitis Viral Induced Wheeze IVAB Non pathway referrals Main duties of the jobThe successful candidate will work within the Children's Hospital at Home team in delivering acute nursing interventions to the children and young people on our Hospital at Home caseload. The role involves undertaking planned clinical assessments following set care pathways and escalating appropriately. The Children's Hospital at Home Team Nurse will be supported by the acute paediatricians and Lead Nurse for Hospital at Home, ensuring that children are kept safe at home whilst on the caseload. The successful candidate will be able to support incident reporting, service development and Quality Improvement and Research opportunities. The Children's Hospital at Home Team Nurse will work within the Community Children's Nursing Service and will support the caseload of complex patients when required. The Children's Hospital at Home Team Nurse will support the overall service delivery by contributing to the end of life on-call rota which is activated when required. Training and support will be provided. The Children's Community Nursing Team offer a rolling education programme, including a full induction for new starters into the team. The Hospital at Home service takes direct referrals from GPs , from A and E and step downs from the inpatient wards. We support early discharges for assessment and monitoring and provide care to meet the needs of the acutely unwell child in the community. We operate from four locality bases across Oxfordshire and our locality teams are responsible for delivering all aspects of our service which includes caseload management, respite sessions, palliative care, acute nursing interventions and special school nursing support. Applicants must have a full valid driving license and access to a car for work. We see our staff as our biggest asset and are committed to supporting career progression within the community. This role is based at a locality base, however we offer a county wide service and travel to other areas/bases is required. This role is full time for 37.5 hours per week. Our core service hours are 8am-8pm. The shift pattern will be a variety of early (8am-4pm) and late shifts (12pm-8pm) including weekends and bank holidays. The on-call palliative rota operates 24hrs/day and is only activated if required. If necessary flexible working adjustments can be considered in line with the Equality Act 2010.
