The Role:As a Family Practitioner in the Family Connections Service, you'll be at the forefront of building meaningful relationships with Kinship Carers in Hampshire. You will provide support to carers on Special Guardianship Orders, helping guardians to achieve the best outcomes for children.The Family Connections Service has recently aligned to our Family Help model, and therefore we have 3 roles available that will each align to one of the following districts:Test Valley Hart & Rushmoor Countywide with a focus on supporting Kinship carers who live outside of Hampshire What you’ll do:Undertaking direct work with children and kinship carers. Co-working with the allocated Social Worker during the assessment process and completing tasks such as foster care supervision, health and safety checklists and back up carer assessments. Focused assessments in specific areas, as agreed with Assistant Team Manager Team Manager and in line with agreed competencies. This includes the adoption and special guardianship support fund applications and special guardianship support plans Planning of specific tasks/activities within the overall Special Guardianship Support Plan and in consultation with the Assistant Team Manager, where required. Facilitating support groups with kinship carers. Delivering training to kinship carers. Offering advice, guidance and signposting to kinship carers. What we’re looking for:Essential Skills, Qualifications and Experience Qualifications: Literacy and numeracy to GCSE level NVQ level 2 IT Skills Ability to meet the travelling requirements of the role.Experience: Voluntary or paid experience of working with children, young people and families in a helping or caring capacity. Experience of using mediation/negotiation skills to achieve an outcome Significant experience of providing services to children/ and or families with complex needs Travel Requirements:You will be required to travel to various locations within Hampshire and the Isle of Wight or other local authority, and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis. Travelling to other parts of the country will also be required. as the role covers a large geographical area.
