You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development.Main duties of the jobWe are seeking a highly motivated and ambitious District Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants.You will undertake clinical skills including venepuncture, wound care, catheterisation and end of life including syringe drivers.You will manage a caseload of patients attached to 1 or 2 GP surgeries. The role will include caseload management of patients with a range of long-term conditions and multiple pathologies. You will provide holistic assessment of health needs, relevant to nursing care and offer health promotion. You will need to have a District Nursing Qualification and experience of managing a team. You will act as a role model, teaching and supporting staff and students in the community.Post holders will be available to work Monday to Sunday between 8am and 8pm.
