We work in partnership with Sussex Partnership NHS Foundation Trust and GPs, providing integrated support pathways for clients, in particular clients who face more complex mental health situations. As a Mental Health Support Coordinator you’ll be co-located and work in partnership with a multidisciplinary team to identify and provide targeted mental health and holistic support.You’ll support clients, from completing initial assessment through to facilitating access to other mental health services. A key part of the role will be to deliver short term recovery-focused sessions with clients, co-creating support plans and goals, and using coaching and motivational interviewing to explore barriers.If you are passionate about providing recovery focused support to enable clients to reach their goals, we’d love to hear from you.
