Job overviewRehabilitation Nurse - Band 5 Full-time - 37.5 hours per week The Rehab unit provides short-term intermediate care rehabilitation to regain and improve the independence of people who might otherwise face unnecessarily prolonged hospital stays or inappropriate admission to hospital or residential care. The unit has a Step-Down function with patients being admitted from hospital. The care is person-centered, focused on rehabilitation, and delivered by a multi-disciplinary team. The Post holder is responsible for managing their own caseload and making onward referrals to appropriate services following an assessment of the patient’s needs.
