The role involves the assessment, diagnosis, treatment, or referral of patients presenting with undifferentiated and undiagnosed conditions. Key Responsibilities Clinical To provide assessment, diagnosis, and treatment at first point of contact by attending to patients according to their needs, either in the Practice or at patients' homes. To support and develop the Practice's same-day emergency care service. To evaluate clinical information from examinations and history-taking and initiate appropriate treatment and/or referral. To signpost appropriately to other services within defined healthcare pathways. To refer patients to appropriate care settings within locally agreed healthcare pathways. To maintain accurate, timely, and systematic documentation of patient interactions and clinical care provided, including adverse events. To offer patients health promotion advice, support for healthy lifestyles, and disease prevention strategies. To provide clinical advice and support to other team members where appropriate. To provide effective chronic disease management for patients, including conducting home visits when required.
